Please attend the surgery with 1 piece of photographic ID (passport/Photo driving licence) and 1 proof of address (utility bill/Paper driving licence). You can complete a registration form in the surgery. All new adult patients will be offered a new patient check with a our Practice Nurse or Health Care Assistant.
It may well take some time for us to receive your full medical records and the doctors therefore feel it is important for all new patients to attend a new patient screening with our practice nurse, who will take your medical and family history, carry out checks such as blood pressure and urine testing and will then arrange for you to see a doctor if appropriate. Failure to attend your new patient health check may effect your registration.
